I'm just saying "I should have double checked you were on the list" goes a lot further as an apology than "the people under me fricked up but I'm in charge so it's my bad". Great story but when someone in charge takes the blame then says "well the people under me messed up but I'm in charge so it's my fault" that kinda defeats the purpose of taking the blame because you're still passing the blame in an offhanded manner. Just fall on the sword, no need to blame your subordinates.
