The fact that you think that clip doesn't show it only shows you don't know how the system works, practically or logistically. A doctor diagnosing a sore throat doesn't say "I get paid more to do a tonsillectomy." They say "this kid needs a tonsillectomy let me refer him to a pediatric OTO." There's no additional money in it for the referring doctor. And practically, the insurance company would require documentation that a tonsillectomy was required, including diagnosed instances of issues in the past 6 months to a couple years.
